Overview

Learn when to consider heart failure patients for advanced therapies. Introduction of a protocol for referral and transfer of heart failure patients for LVAD or heart transplant. Explore commonly missed heart failure quality measures. Learn how to electronically refer a patient to cardiac rehab.

It is anticipated that it will take 60 minutes to fully participate and complete this activity. In order to complete this activity and claim credit you must:

1) Complete the pre test.

2)Watch the Powerpoint presentation and visit the links.

3) Take and pass the post test with a grade of 80% or better

4) Complete the course evaluation

Objectives
At the conclusion of this activity, learners will be able to:

  1. List four criteria for when to refer a patient for advanced heart therapies: LVAD or Heart Transplant
  2. List the steps in the process for referring a patient to another hospital for a higher level of care
  3. Identify the AMC Intranet location for heart failure resources: AMC Protocol for Referral for Advanced Heart Failure Therapies (LVAD, Heart Transplant) Current Heart Failure Treatment Guidelines
  4. State which classification of heart failure patients should receive SGLT2i medications
  5. State the recommended lab monitoring following initiation or increased dose of mineralocorticoid antagonist
  6. State the benefit of hydralazine and isosorbide dinitrate therapy for HFrEF in self-identified as Black or African American
  7. State two benefits of follow-up within 7 days of discharge for HF patients
  8. State two benefits of cardiac rehab
  9. Demonstrate how to complete a cardiac rehab referral order in Epic
